Parents and New Athletes:

We have new forms.  Please open the Word document to the left and fill the form out online (except physician exam form).  Once you have finished please print the completed forms, sign where appropriate, get your physical done by a physician, and send all of the forms in.  Please understand that for liability reasons no athlete will be allowed to participate in any type of team training until that athlete's entire physical packet is completed.

Please complete all sections!

Please include a copy of the front and back of your current insurance card when you mail the information in.
